3 Affidavit of two (2) disinterested persons who might have witness/knowledge about the child’s birth Public Attorney’s Office or Notary Public, Mayor’s Office
4 Marriage Contract/Certificate, if married Philippine Statistics Authority (PSA)
5 Any Two of the following documents where we can base the date and place of birth of the child below
6 Baptismal Certificate Church where the child was baptized
7 Voter Certification /Record COMELEC
8 School Records (Form 137 or Official Transcript of Records School attended
9 Yellow Card (Medical Certificate) Clinic
10 Barangay Certification Barangay Hall
11 Valid ID’s or any permanent documents Concerned government agencies
12 Affidavit of the applicant declaring among other things the facts of birth and the reason why said birth was not recorded / registered in the city/municipality where it occurred. The affidavit shall be attested by two (2) witnesses and shall serve as an application for registration. a.i.8. Public Attorney’s Office or Notary Public
